Ten-year thesis

UnitedHealth makes money taking risk on health costs and then earning more by managing those costs through Optum. Scale in networks, data, and government programs (Medicare Advantage) is the moat. A ten-year hold is a bet that US healthcare remains a private-public hybrid and that UNH remains the most competent operator in it. Earnings quality has been excellent for a long time; political and legal overhangs are why the hold score is not 'forever' without a discount.

The moat

Scale in provider networks, government program expertise, and vertical integration into care delivery. Switching costs for employers are real but not absolute. Political risk is part of the moat (complexity) and part of the threat.

Switching costs72

Pain, risk, or retraining required to leave.

Network effects68

The product gets better as more people use it.

Intangibles & IP78

Patents, data, licenses, process knowledge, regulation.

Scale advantage92

Fixed costs spread over a volume rivals cannot match.

Cost advantage80

Structural ability to be the low-cost producer.

Brand70

Pricing power from trust, habit, or identity.

Tech overlay

Optum is a data-and-software business hiding inside an insurer. A tech owner can underwrite that layer. The stock still lives or dies on medical-cost ratios and Washington.

What can break it

  • Medicare Advantage rate cuts and utilization spikes
  • Antitrust and political targeting
  • Optum legal/regulatory issues

Owner earnings yield 4.6%. Yield is judged against quality, not against a single hurdle — a fortress can be fairly priced at a lower yield than a fragile name.
